Annonc\u00e9e ce lundi, cette exp\u00e9rience, r\u00e9alis\u00e9e avec succ\u00e8s par une \u00e9quipe de scientifiques suscite un grand espoir pour la m\u00e9decine mais pose aussi des questions \u00e9thiques.<br>Cette nouvelle avanc\u00e9e a \u00e9t\u00e9 annonc\u00e9e dimanche, lors d\u2019une r\u00e9union de l\u2019Association am\u00e9ricaine pour la promotion de la science (AAAS), \u00e0 Austin, au Texas. L\u2019an dernier, des chercheurs du Salk Institute, en Californie, avaient d\u00e9j\u00e0 annonc\u00e9 les premiers embryons hybrides \u00abhomme-cochon\u00bb, mais l\u2019exp\u00e9rience avec les moutons est plus prometteuse.<\/p>\n\n\n\n<p><strong>Pourquoi ?<\/strong><\/p>\n\n\n\n<p>L\u2019int\u00e9r\u00eat de faire prosp\u00e9rer des cellules humaines dans le corps d\u2019une autre esp\u00e8ce est \u00e0 terme de pouvoir transplanter des organes provenant d\u2019animaux chez des humains. Et ainsi de pallier la p\u00e9nurie d\u2019organes : avec les progr\u00e8s de la m\u00e9decine, les demandes de greffes augmentent plus vite que le nombre de donneurs. Au 1er janvier 2016, en France, 14 471 patients \u00e9taient inscrits en liste d\u2019attente d\u2019un organe, un chiffre en perp\u00e9tuelle augmentation. Entre 400 et 600 personnes inscrites sur cette liste d\u00e9c\u00e8dent chaque ann\u00e9e dans notre pays, la majorit\u00e9 d\u2019entre eux ayant attendu en vain un rein ou un foie.<\/p>\n\n\n\n<p>Mais pourquoi ne pas transplanter directement un organe d\u2019une autre esp\u00e8ce sur un humain ? Parce que si les moutons ou les porcs ont des organes similaires aux humains, ce transfert direct d\u2019organes, appel\u00e9 x\u00e9notransplantation, a tr\u00e8s peu de chance de r\u00e9ussir. Pour contourner cet obstacle et \u00e9viter les rejets, l\u2019id\u00e9e est donc de cr\u00e9er une chim\u00e8re homme-animal, qui serait l\u2019 \u00abh\u00f4te\u00bb d\u2019organes transplantables \u00e0 l\u2019avenir. Lors de cette exp\u00e9rience, contrairement \u00e0 la pr\u00e9c\u00e9dente, les scientifiques ont privil\u00e9gi\u00e9 des embryons d\u2019ovid\u00e9s plut\u00f4t que de bovid\u00e9s, les premiers se d\u00e9veloppant plus facilement par f\u00e9condation in vitro (FIV) que les seconds, et leur implantation dans un animal adulte ayant donc beaucoup plus de chance d\u2019\u00eatre un succ\u00e8s.<br>Cette nouvelle exp\u00e9rience a donc pour objectif de cr\u00e9er des pancr\u00e9as humains dans des moutons, ce qui pourrait permettre de gu\u00e9rir le diab\u00e8te de type 1, qui r\u00e9sulte de la destruction des cellules du pancr\u00e9as s\u00e9cr\u00e9tant l\u2019insuline.<\/p>\n\n\n\n<p><strong>Comment?<\/strong><\/p>\n\n\n\n<p>Pour que des organes humains se d\u00e9veloppent chez des animaux, il faut utiliser des cellules souches. Pr\u00e9sentes chez les embryons, rares chez les adultes, elles ont le m\u00e9rite de n\u2019avoir pas de fonction d\u00e9termin\u00e9e et de pouvoir remplir le r\u00f4le que souhaite le m\u00e9decin.<br>Pour obtenir un pancr\u00e9as humain dans un mouton, il faut d\u2019abord supprimer dans l\u2019embryon du mouton la portion d\u2019ADN qui commande le d\u00e9veloppement d\u2019un pancr\u00e9as. Pour cela, les chercheurs ont utilis\u00e9 CRISPR\/Cas9, un outil r\u00e9volutionnaire d\u2019\u00e9dition du g\u00e9nome. On introduit ensuite dans l\u2019embryon les cellules souches humaines qui, elles, \u00ab pr\u00e9voient \u00bb la formation d\u2019un pancr\u00e9as. L\u2019embryon est transplant\u00e9 une dizaine de jours plus tard dans le corps d\u2019une brebis.<\/p>\n\n\n\n<p><strong>Quelles \u00e9tapes \u00e0 venir ?<\/strong><\/p>\n\n\n\n<p>Dans les embryons de moutons modifi\u00e9s, une cellule sur 10 000 est humaine, ce qui est dix fois sup\u00e9rieur au nombre de cellules que l\u2019on trouvait chez les cochons modifi\u00e9s. Mais trop peu pour une greffe d\u2019organe r\u00e9ussie. Selon Pablo Ross, chercheur \u00e0 l\u2019universit\u00e9 de Californie \u00e0 Davis, membre de l\u2019\u00e9quipe, 1 % de cellules humaines sont n\u00e9cessaires.<\/p>\n\n\n\n<p>Les r\u00e8gles actuelles interdisent aux laboratoires de permettre la survie de ces hybrides au-del\u00e0 de 21 jours, 28 jours dans le cas du cochon. Les chercheurs ont demand\u00e9 \u00e0 pouvoir \u00e9tendre \u00e0 70 jours l\u2019exp\u00e9rimentation, pour voir si les cellules humaines peuvent vraiment permettre de cr\u00e9er un organe. Cit\u00e9 par The Guardian et The Telegraph, Hiro Nakauchi, chercheur \u00e0 l\u2019universit\u00e9 Stanford de Californie et membre de l\u2019\u00e9quipe, croit \u00e0 la r\u00e9ussite de cette entreprise dans cinq ou dix ans.<\/p>\n\n\n\n<p><strong>Quels risques et quelles limites ?<\/strong><\/p>\n\n\n\n<p>Selon Robin Lovell-Badge, de l\u2019Institut Francis Crick \u00e0 Londres, la m\u00e9thode choisie n\u2019emp\u00eachera pas les rejets. \u00ab M\u00eame s\u2019ils r\u00e9ussissent \u00e0 remplacer tous les types de cellules pancr\u00e9atiques chez le mouton par des cellules humaines, les vaisseaux sanguins du pancr\u00e9as proviendront des moutons \u00bb, a-t-il averti. Les scientifiques devront \u00e9galement s\u2019assurer que les virus des animaux sont absents du futur greffon.<\/p>\n\n\n\n<p>Il faudra aussi r\u00e9gler la question des moyens : les Etats-Unis interdisent pour le moment le financement public d\u2019hybrides animaux-humains. Le moratoire pourrait cependant \u00eatre lev\u00e9.<\/p>\n\n\n\n<p>Les chercheurs sont conscients de l\u2019aspect controvers\u00e9 de leur travail. Lors de la pr\u00e9sentation de la recherche dimanche, Hiro Nakauchi a expliqu\u00e9 que les chercheurs s\u2019attacheront \u00e0 emp\u00eacher la prolif\u00e9ration de cellules humaines dans le cerveau ou les organes sexuels des animaux.
